About the Role
We are looking for a Practice Transformation Specialist to join Aledade and make a direct impact on patient care. This role serves as the primary interface between headquarters and healthcare providers, supporting practice transformation and population health initiatives. The position requires up to 50% domestic travel within the United States, with remote work available outside of travel periods.
Key Responsibilities
- →Implement products and execute ACO PY0 initiatives in coordination with market leadership
- →Support practice onboarding experience including introduction to Aledade technologies and coaching
- →Ensure practices meet goals set in mutually developed PY0 Milestones and Practice Transformation Plans
- →Train office staff and physicians on processes and technologies using Value Based Care Curriculum
- →Assess practice and staff training needs and develop lasting relationships with healthcare providers
- →Listen to practice concerns and relay important information to management for improvement opportunities
Requirements
- ✓4-6 years of significant work experience in medical practice management
- ✓3 years of experience with Electronic Health Records for clinical/practice management processes
- ✓Excellent oral and written communication skills including public presentation abilities
- ✓Strong organizational, project management, and team-building skills
- ✓Familiarity with process mapping and work-flow analysis tools
- ✓Excellent computer skills and willingness to learn additional software applications
- ✓Ability to travel 10-50% of the time (varies by market)
- ✓Comfort working remotely or from home anywhere within the United States
