About the Role
We are looking for a Practice Transformation Specialist to join Aledade and make a direct impact on patient care. This role serves as the primary interface with healthcare providers, supporting practice transformation and population health initiatives. The successful candidate will be passionate about improving patient care while reducing costs.
Key Responsibilities
- →Implement products and execute ACO initiatives with direction from Market Leadership
- →Support wellness visits, transitions of care, and active management programs
- →Ensure practices meet goals set forth in Practice Transformation Plans
- →Train office staff and physicians on processes and technologies
- →Assess practice and staff training needs on an ongoing basis
- →Develop relationships with physicians, providers, and healthcare executives
- →Listen to practice concerns and relay information to management for improvement opportunities
Requirements
- ✓Must be a resident of Iowa
- ✓Graduate of an accredited college or university
- ✓4-6 years of relevant work experience in medical practice management (may substitute for education)
- ✓3 years of experience with Electronic Health Records for clinical/practice management
- ✓Excellent oral and written communication skills
- ✓Strong organizational and project management abilities
- ✓Familiarity with process mapping and work-flow analysis tools
- ✓Excellent computer skills and willingness to learn new software
- ✓Ability to travel to practices within Iowa markets up to 20% of the year
