About the Role

We are seeking a compassionate and skilled Nurse (RN or LPN) in Shiprock, NM to provide patient care in accordance with physician-established care plans. This is a 1099 contract opportunity with care provided at the patient's residence, requiring 13 hours of weekly nursing care. You will work one-on-one with patients to deliver personalized care in a comfortable home environment.

Key Responsibilities

• →Execute physician prescribed plans of care
• →Complete compliant documentation of care provided
• →Manage and administer prescribed medication, treatment, and therapies
• →Conduct patient assessments and coordinate care
• →Perform various activities associated with daily living
• →Monitor vitals and GI intake and output
• →Assess skin integrity and administer wound care
• →Monitor for changes in patient condition and disease progression
• →Communicate with family members, physicians, and case managers
• →Provide education and training to patients and their families
• →Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants
• →Perform other personal care services as necessary
